INTRODUCTION: Introduction: Osteoarthritis is the most frequent form of joint disease, it is a progressive condition that affects the synovial joints, leading to anomalies in the joint tissues and causing functional impairment, pain and stiffness in patients. Total knee arthroplasty (TKA) is a surgery performed worldwide, often based on the premise that the use of tourniquets is related to better intra- and postoperative events. However, there are several studies that describe worse outcomes and complications, both local and systemic. AIMS: Objectives: The purpose of this progress report focused on the possible consequences of tourniquet use is to investigate possible differences in length of stay, need for transfusions, and postoperative complications that include hematoma, arthrofibrosis, wound dehiscence, infections, DVT, pulmonary thromboembolism, acute myocardial infarction (AMI) and deaths in the first six months following surgery. MATERIALS AND METHODS: Materials and Methods: We conducted a retrospective observational study, analyzing the medical records of patients who underwent total knee arthroplasty between 2017 and 2022 in the same hospital, with surgeries performed by the same surgical team. Patients were divided into two groups based on whether or not a tourniquet was used during surgery. RESULTS: 104 patients underwent surgery during the selected period, 94 of whom were included in the study. There was no significant difference in surgical time, length of stay, need for transfusions, new surgeries to drain postoperative hematoma, to release arthrofibrosis, wound dehiscence, superficial and deep infection, DVT, PTE, AMI and deaths between the two groups. FINAL CONSIDERATIONS: It was evident that the difference in surgical and hospitalization time for patients was not statistically significant, as were other variables analyzed and possible complications.
